This web application is for informational purposes only and not for health purposes. Its use does not constitute in any case a service for medical diagnosis, emergency care or prescription of pharmacological treatment.
The CHRONIBERIA index is the result of a collaborative project developed in different areas of Spain and Portugal, which was designed to find out the variables that influence mortality after discharge and to develop a prognostic index adapted to the current care needs of chronic patients in the area of internal medicine.
To get to the CHRONIBERIA index, data from two previous prospective studies were used: CRONIGAL (developed in the Sanitary Area of Santiago de Compostela) and PLUPAR (developed in Aragon). In addition, new data from two similar cohorts recruited from two hospitals in Portugal were added: Pedro Hipano Hospital (in Matosinhos) and Santo Antonio Hospital (in Porto).
All the clinical units that participated in this project were made up of doctors with experience in the field of chronic patient management.
3.1. ABOUT CHRONIBERIA
CHRONIBERIA presents a simplified and improved index to predict the survival prognosis of the chronic patient, as well as one-year mortality in chronic patients after discharge. This was done based on the data provided by users, who are patients with chronic diseases (obstructive pulmonary disease, atrial fibrillation, liver disease, etc.).
Users must provide the data by filling in a series of variables that CHRONIBERIA will take into account.
The variables that constitute the new CHRONIBERIA index are a series of factors: being older, being male, having a low BI (Barthel index) score, AF (atrial fibrillation) or an active neoplasia. These could be essential in identifying high-risk chronic patients.
CHRONIBERIA aims to contribute to reducing the impact of chronic diseases on the quality of life of patients.
4. INTELECTUAL AND INDUSTRIAL PROPERTY
Both FIDIS and its related entities reserve all their intellectual and industrial property rights over the content and information of CHRONIBERIA. By way of example and not limitation: the code, images or texts are the joint property of FIDIS, SERGAS and the USC, or they have the corresponding authorizations for their use, without prejudice to intellectual and industrial property rights, with those that the project collaborators can count on.
